This gentleman had prior surgery in another country 8 years ago and now presented with severe nasal obstruction causing sleep difficulties.  He underwent revision nasal surgery primarily to improve his nasal breathing.  However, this case shows how closely linked cosmetic and functional problems can be.

 

Details:

Notice before revision surgery, the middle portion of the patient’s right side of the nose is collapsed and the patient’s right tip of the nose is droopier than the left. In the post-op photo, the right nose collapse has been filled in and the tip is more symmetric.
The patient did not have any desire to make any significant changes to the profile. The nose after surgery is less pointy.
On the base view, note the improvement in the tip of the nose, now more symmetric, and the middle of the nose (columella) is straighter.
